As patients move through their care journey, from acquisition and onboarding, through ongoing care, to their financial experience, there should be consistent, reliable communication touchpoints. Appointment automation can help deliver on these. Better engagement with patients means better care plan adherence, better outcomes, and higher patient satisfaction. While appointment reminders are critical for making sure patients keep their visits, there is far more to automation than just that. This page walks through the three capabilities that turn a reminder tool into a whole-journey engine (referral management, post-visit follow-up, and recall outreach), and shows what two Artera customers achieved when they extended their existing deployments beyond reminders.
Automated Referral Management
Nearly 50 percent of patient referrals are never completed.
Communication breaks down between patients and providers. Patients forget to schedule. Loops close silently and never close correctly. That is where a real referral management software layer changes the outcome, by keeping every handoff open, time-stamped, and re-engaged until the visit actually happens.
Artera Referrals Management addresses these breaks directly. It simplifies the patient-to-provider conversation, streamlines the workflow around each referral, and cuts missed appointments and patient leakage across specialties. It also contributes to preventing hospital readmission by ensuring patients get the specialist care they need immediately after hospitalization. Multi-step referral logic now runs on Flows Agents, Artera’s rules-based workflow engine, so complex handoffs do not require a separate build. The referral management workflow writes back into your EHR through the same bidirectional connection Artera already uses for reminders, so care coordinators keep working inside their existing systems. Coupled with Artera’s automation engine, referrals turn from a leak into a closed loop, a concierge patient experience across the whole care journey.
Post-Visit Follow-Ups
Many Artera customers send automated messages after a visit, whether that is a medication instruction, a directions reminder, a feedback prompt, or a request to leave a review on Google. Artera’s Conversation Builder handles this with multi-step decision trees that resolve most inquiries without a live staff member, and route the handful that need one to the right person. AI Agents, deployed by voice or text, now handle even more of that back-and-forth directly, with Conversation Builder’s decision trees running underneath so nothing about the workflow you already trust changes.
At Sansum Clinic, 95 percent of Conversation Builder conversations that patients engaged with were resolved without staff intervention. Natural language understanding lets the platform interpret how patients actually reply, in the phrasing they actually use, so the exchange stays conversational rather than scripted. Artera customers have adapted this same conversational AI in healthcare pattern to help new pediatric patients schedule, to follow up on cancellations and no-shows, and to guide patients into telehealth visits without a manual phone call in the middle.
Recall Automations
The days are over of asking patients to complete a self-addressed postcard to schedule their next annual visit, or of having staff manually work a call list to book each recall. Automated patient recall workflows replace those tasks with a nudge that reaches each patient in the medium they actually respond to, and they free front desk staff for higher-value work like patient check-in and answering questions in the lobby. Artera Recalls handles scheduling for annual wellness visits, dental hygiene, medical supply reorders, and every other recurring recall your team runs today.
The nudge itself can now come from an AI Agent by voice as well as by text, for patients who respond better to a call. Harmony Co-Pilots translate outreach into any of Artera’s 109 supported languages before it sends, so multilingual patient populations get the recall in their preferred language without a staff-driven translation step. What that combination unlocks: fewer missed appointments, higher patient retention, lower no-show rates, and staff time recovered that would otherwise go to manual dialing. Recalls handled well are one of the fastest paths to a lower no-show rate a practice can turn on.
Why You Should Be Using More than Just Appointment Automation
Automation is a key component of modern patient engagement, but it does not need to be a heavy lift. Artera Harmony comes with predefined automated conversation templates for common tasks and workflows, so extending scope means adapting a template your team already has, not integrating a new tool. Harmony Co-Pilots make the humans running these automations move faster too, with AI-drafted message shortening, translation into any of Artera’s 109 supported languages, and conversation summaries that give staff a full picture of the exchange in a single glance. That is what modern healthcare workflow automation actually looks like: one platform, one template library, and every touchpoint humming without a headcount lift. Two Artera customers already proved this at very different scales.
Automation is a key component of leading the charge to create modern patient engagement, but it doesn’t need to be a heavy lift. At Artera, we are helping you to get started by providing numerous features and predefined automated conversation templates for common tasks and workflows to supplement your team AND increase patient throughput.
Proven in practice: two health systems, two outcomes
Two independently published results, from two very different organizations, show what happens when appointment automation stops at reminders and what happens when it does not.
Houston Methodist cuts readmissions with post-discharge texting
When Houston Methodist wanted to know whether automated post-discharge texting actually changed outcomes, they did not rely on a vendor case study. They ran a clinical study, tracked close to 80,000 patients, and published the results in a peer-reviewed BMJ Open study.
Patients who received Artera-powered post-discharge texts had 29 percent fewer 30-day readmissions and were 27 percent less likely to be readmitted than patients who did not receive the texts. Revisit rates dropped 20 percent. HCAHPS scores rose across every domain measured, up as much as 7.4 points on individual dimensions and 2.8 percent on the overall hospital rating.
The mechanism is the same referral and follow-up automation described above, aimed at the highest-risk moment in the care journey: the days right after discharge. Full findings and methodology are documented on the Houston Methodist post-discharge case study page.

Beauregard Health System closes gaps in care in six weeks
Beauregard Health System is a rural provider founded in 1950, with 35-plus providers across 10 satellite clinics serving more than 45,000 patients in Beauregard Parish, on the MEDITECH Expanse EHR. Beauregard has run Expanse Patient Connect Powered by Artera since January 2021 and added Conversation Builder and Conversation Flows in June 2024.
Manual, cold-call outreach for preventive screenings was not scaling. Population Health Coordinator Nicole Clanton adapted Artera’s existing campaign templates, worked with seven primary care providers to align each one to their specific workflow, and had full campaigns live in six weeks.
Over a two-month window, Beauregard closed 18 percent of its open HEDIS Breast Cancer Screening (BCS-E) gaps and 13 percent of its open HEDIS Colorectal Cancer Screening (COL-E) gaps, tracking against the eight CMS quality measures the health system reports on, including hemoglobin A1C, mammograms, and colonoscopies. Average outreach call time dropped from five to ten minutes down to 30 seconds, saving two weeks of call center time.
“From conception to deployment, these campaigns came together so quickly, it really is so easy to learn Conversation Flows! I have been working in population health for several years, and nothing has been as effective or seamless as Artera, a true all-in-one solution.”

Frequently asked questions
The questions below are the ones Directors of Patient Access, COOs, and CIOs actually ask when deciding whether to expand an existing Artera deployment beyond reminders.
How does Artera route referral management workflows back into Epic, Oracle Cerner, or MEDITECH without duplicate data entry for our care coordination team?
Referral management workflows write back through the same bidirectional API, FHIR, or HL7v2 connection Artera already uses for appointment reminders, so a completed referral, a missed handoff, or a rescheduled visit updates your EHR directly. Care coordinators keep working inside Epic, Oracle Cerner, or MEDITECH; nothing is re-keyed by hand.
What is the typical timeline to deploy recall and follow-up automation on top of an existing MEDITECH Expanse Patient Connect or Epic deployment?
Typical timeline runs four to six weeks when you are extending an existing Artera deployment, close to the six weeks Beauregard Health System needed to launch its recall campaigns on MEDITECH Expanse Patient Connect Powered by Artera. Most of that time goes to adapting existing message templates, not building integration from zero.
How does Artera measure HEDIS BCS-E and COL-E gap closure after a recall or outreach campaign goes live?
HEDIS BCS-E and COL-E gap closure is measured the same way Beauregard Health System tracked it: comparing overdue patients on your quality measure list before the campaign against confirmed completions after, typically over a defined window like the two-month period Beauregard used for its mammography and colorectal screening campaigns.
Does extending automation into referrals and recalls change our no-show rate reporting, or does it stay inside the same Harmony analytics we already use?
No-show rate reporting stays inside the same Artera Harmony analytics dashboard you already use for reminders. Referral and recall automation add new event types, referral closed, recall completed, screening scheduled, to that same dataset instead of standing up a separate report your team has to reconcile by hand.
Can post-visit follow-up and recall automation run through Flows Agents and AI Agents at the same time, or do we have to pick one?
Post-visit follow-up and recall automation can run on Flows Agents, AI Agents, or both together; they are not an either-or choice. Flows Agents handle the rules-based, multi-step conversations you already use in Conversation Builder, while AI Agents add voice or text handling for the parts of that conversation that need more flexibility.
How does Artera Referrals Management prevent the 30-day readmission risk that shows up when a referral loop closes late or not at all?
Artera Referrals Management prevents that risk by keeping the referral loop visible and time-stamped instead of closing silently. The same automation logic Houston Methodist’s post-discharge texting program used to cut 30-day readmissions by 29 percent applies here: a patient who never schedules the follow-up referral gets flagged and re-engaged before the 30-day window closes.
What did it take for a health system the size of Beauregard to go from manual call lists to automated recall campaigns in six weeks?
Beauregard’s six-week timeline started with adapting Artera’s existing Conversation Flows templates rather than building new ones, then having Population Health Coordinator Nicole Clanton work directly with seven primary care providers to align each campaign to their workflow. No new EHR integration was required since Expanse Patient Connect Powered by Artera was already live.
Extend automation you already have
Appointment reminders get patients to the visit. Referral management, post-visit follow-up, and recall automation are what keep them inside your system afterward: closing a referral loop before a readmission risk sets in, clearing a HEDIS screening gap, or freeing a population health coordinator from a manual call list. Houston Methodist and Beauregard Health System did not add headcount to get those results. They extended automation they had already deployed. If appointment reminders are the only piece of Artera Harmony your organization has turned on, the rest of the care journey, and the outcomes attached to it, is one build-out away.
